Caring From NICU to Adulthood:
Meet the 2025 OCHCH Home Health Award of Excellence Winner - Traci Osborne, RN

Professional care for individuals with specialized needs demands knowledge, compassion, and dedication. Providing care consistently from the neonatal intensive care unit (NICU) into adulthood for one patient requires even more.
At Home Care Network (HCN), we are honored to celebrate Traci Osborne, RN, who was named the 2025 “Home Health Award of Excellence” winner by the Ohio Council on Homecare and Hospice (OCHCH). This award recognizes the outstanding courage, love, and dedication she has delivered to her patients and her unwavering support for their families throughout a remarkable 25-year career in home health.
Nominated by her supervisor, Madison Miller, Northern Ohio Regional Administrator at HCN, Traci’s co-workers know first-hand that her success stems from a special, instinctive gift. “Home health nursing is not for everyone; we go into people’s homes and care for them in a much different manner than in a hospital setting,” Madison explains. “Traci is incredibly intuitive and notices little things others might miss—like an unspoken concern or a subtle change in mood—and responds with compassion and grace. She is the kind of person who makes you feel safe, seen, and supported, making her not only an exceptional nurse but an extraordinary human being.”
Traci’s commitment goes far beyond basic requirements. Her first priority is always her patients’ care and well-being, a principle that has helped families adjust and grow with confidence and independence over the decades.
One particularly powerful example of Traci’s impact is her decades-long relationship with a patient she has cared for since infancy: What started as medical support evolved into a profound mentor-like relationship. Traci has not only taught her patient how to manage their condition, but also actively facilitated their independence. She has supported her patient through significant adolescent milestones, from attending summer camps to participating in church social events. This reliable presence has given her patient the confidence to live a full life and, most recently, to move out of the family home to seek more freedom.
For Traci, this deeply personal approach is at the heart of what she does. She advises other home health nurses to:
“realize it’s not just the client you are caring for, it’s the whole family. Every day may be a different emotion for your client depending on what they have gone through—don’t take things they say personally.”
Madison beautifully summarizes Traci’s legacy: “Traci embodies what it truly means to be a caregiver, not just in her skillset but also in her heart. She champions her patient’s journey, celebrates their milestones, and ensures they feel seen and supported each step of the way.”
We are incredibly proud to have Traci as part of the HCN family. Her character, work ethic, and compassion reflect the goals we strive for as a company. And to no one’s surprise – Traci’s reaction upon receiving the honor was as genuine and lovely as she is. “This is so unexpected – what a happy surprise! It feels odd to get an award for something that I consider a part of my life, but it is quite a compliment that I truly appreciate!” Looking forward, Ms. Osborne doesn’t see herself leaving home health anytime soon, stating: “I haven’t lost my passion for this line of work and God hasn’t told me to quit yet, so here I am!”
